| The Identification form (B-10)
collects basic organizational and transit service information about transit agencies
filing National Transit Database (NTD) reports. Internet Reporting uses this information
to generate the appropriate forms for the reporter, based on the number of vehicles
operated by mode and type of service (TOS), capital improvements for modes of
service not yet in operation, contractual agreements with transit agencies filing
their own NTD reports, a Nine or Fewer Vehicles Waiver request and the size of
the urbanized area (UZA). This form is required for all transit agencies. |
| The Contacts form (B-20) identifies
key personnel at the transit agency responsible for NTD reporting. The form collects
information on how to reach the chief executive officer (CEO), and NTD, safety
and security contacts. Transit agencies may also identify additional personnel
to receive NTD communications, if desired, by contacting their validation analyst.
This form is required for all transit agencies. |
| The Contractual
Relationship form (B-30) identifies the buyers and sellers of purchased transportation
(PT) services and provides key financial and operating data for the purchased
service. This form is required for all transit agencies with contractual
relationships (buying or selling) for purchased transportation (PT) services except
those filing a Nine or Fewer Vehicles Waiver. |

The
Sources of Funds - Funds Expended and Funds Earned form (F-10) collects data on
the transit agency's sources of funds for transit operations and capital by funding
category. The funding categories cover sources generated by the transit agency
and from Federal, state and local governments. For each category of funds, the
form collects the following data by original revenue source:
- The total amount earned
- The amount applied for operating expenses
- The
amount applied for capital expenses.
This
form is required for all transit agencies except those filing a Nine or Fewer
Vehicles Waiver. |
| The Uses of Capital
form (F-20) identifies the funds expended for capital projects by type of project,
and by project need as either an improvement for existing transit services or
for expansion of transit services. The required data for uses of capital funds
are reported on one form by mode and type of service (TOS). This form
is required for all transit agencies except those filing a Nine or Fewer Vehicles
Waiver. |
| The Operating Expenses
form (F-30) identifies total operating expenses by object class and function.
The required data use standard expense object classes (line items) detailed by
function (activity performed), as specified in the Uniform System of Accounts
(USOA). Transit agencies complete, by mode, separate forms for directly
operated (DO) and purchased transportation (PT) services. This form is
required for all transit agencies except those filing a Nine or Fewer Vehicles
Waiver. |
| The Operating
Expenses Summary form (F-40) is a system wide summary of the operating expenses
reported, by mode and type of service (TOS), on the Operating Expenses form (F-30).
The operating expenses are summarized by function and expense object class. The
summarized data also includes reconciling items to handle items where accounting
practices vary due to local ordinances and conditions such as depreciation, interest
payments and leases. This form is required for all transit agencies except
those filing a Nine or Fewer Vehicles Waiver. |
| The Operators' Wages form
(F-50) provides a detailed breakdown of hours worked and wages paid to employees
functioning as operators. Required data are categorized by time classifications
for operating and non-operating paid work time. The total expenditures on this
form equal expense object class 501.01 operators' salaries and wages on the Operating
Expenses form (F-30). This form is required for all transit agencies
with 150 or more vehicles operated in annual maximum service (VOMS) in a directly
operated (DO) mode except demand response (DR) and vanpool (VP) modes.
Transit agencies filing their first National Transit Database (NTD) report and
those filing a Nine or Fewer Vehicles Waiver are not required to complete this
form. |
| The
Stations and Maintenance Facilities form (A-10) reports passenger stations and
maintenance facilities. This form is required for all transit agencies
except those filing a Nine or Fewer Vehicles Waiver. Transit agencies complete
separate forms for directly operated (DO) and for purchased transportation (PT)
services by mode. |
| The Transit Way
Mileage form (A-20) collects mileage data on all fixed route and fixed schedule
modes. This form is required for all transit agencies operating on fixed
guideway (FG). Transit agencies complete separate forms for directly operated
(DO) and for purchased transportation (PT) services by mode. The form is not applicable
to ferryboat (FB), demand response (DR), jitney (JT), publico (PB), and vanpool
(VP) modes. |
| The Revenue
Vehicle Inventory form (A-30) reports the revenue vehicle inventory at the end
of the fiscal year and identifies the characteristics of the vehicles in the fleet.
This form is required for all transit agencies. Transit agencies complete
separate forms for directly operated (DO) and for purchased transportation (PT)
services by mode. |
| The Service form (S-10) provides
data on the transit service supplied by the transit agency and the transit service
consumed by passengers. This form is required for all transit agencies
except those filing a Nine or Fewer Vehicles Waiver. Transit agencies complete
separate forms for directly operated (DO) and for purchased transportation (PT)
services by mode. |

The Fixed Guideway Segments form
(S-20) details segments of fixed guideway (FG) facilities in order to determine
directional route miles (DRM) for service operated and for eligibility in FTA
Federal funding programs. Internet Reporting generates the data requirements
for this form based on the mode of service operated. The data requirements vary
by three modal groupings: - Bus
(MB) mode
- Trolleybus (TB) mode
- All
other fixed guideway (FG) modes.
This
form is required for all transit agencies that operate service on fixed guideway
(FG) facilities. Transit agencies complete separate forms for directly operated
(DO) and purchased transportation (PT) services by mode. |
| The Employees form (R-10)
reports data on the number of transit agency employees (person count) and their
total work hours. Data are reported by mode for directly operated (DO) service
only. The employee data are reported for operating labor (which is reported on
the Operating Expenses form (F-30) by function) and for total capital labor.
This form is required for directly operated (DO) service only. One form is
completed for each directly operated (DO) mode. |
| The Maintenance
Performance form (R-20) reports data on revenue vehicle system failures and hours
spent on inspection and maintenance by the transit agencys service personnel.
Data are reported on one form for all modes for directly operated (DO) service
only. This form is required for directly operated (DO) service only.
One form is completed for each directly operated
(DO) mode. |
| The Energy Consumption
form (R-30) reports revenue vehicle fuel consumption. There are two forms tailored
to report fuel consumption for rail and non-rail modes. Data are reported by mode
for directly operated (DO) service only. This form is required for directly
operated (DO) service only. Complete one form for directly operated (DO) rail
modes and one form for directly operated (DO) non-rail modes. |
| The
Federal Funding Allocation Statistics form (FFA-10) summarizes data used in apportioning
funds for the Federal Transit Administration's (FTA) Urbanized Area Formula Program
(Section 5307) and the Capital Program for Fixed Guideway Modernization (Section
5309). This form is required from transit agencies in or serving urbanized
areas (UZAs) with 200,000 or more population using 2000 US Bureau of the Census
information. Transit agencies complete separate forms for directly operated (DO)
and for purchased transportation (PT) services by mode. |
| The
CEO Certification form (D-10) is an NTD form, approved by the transit agency's
chief executive officer (CEO) that endorses and attests to the accuracy of the
data submitted in the NTD Annual report. This
form is required for all transit agencies except those filing a Nine or Fewer
Vehicles Waiver. |

| The Mode Service
Operated form (MR-10) collects the number of vehicles operated in maximum service
(VOMS) by the transit agency. Internet Reporting uses this information to generate
the appropriate ridership activity forms for the transit agency. This form is
completed once at the beginning of the calendar year. This form is required
annually for all transit agencies except those filing a Nine or Fewer Vehicles
Waiver. |
| The Ridership Activity
form (MR-20) collects monthly information regarding transit service provided by
the transit agency. These data provide FTA with monthly trends in passenger usage
and service levels. This form is required for all transit agencies. |

| The
Safety and Security Setup form (S&S-10) is used to identify the modes and
types of service operated by the transit agency. Internet Reporting uses this
information to generate the appropriate safety and security forms for the system.
This form is completed annually at the beginning of the calendar year. This
form is required for all transit agencies. |
|
The
Safety and Security CEO Certification form (S&S-20) is a letter, approved
by the transit agencys chief executive officer (CEO) that certifies and
attests to safety and security (if applicable) data submitted in the previous
years reports. This
form is required for all transit agencies except those filing a Nine or Fewer
Vehicles Waiver. |

The Security Configuration
form (S&S-30) is used to collect information on the number and type of police
and/or security personnel used to provide security at a transit agency. This form
is completed annually at the beginning of the calendar year. This form
is required for all transit agencies. |
|
The Reportable Incident
Reporting form (S&S-40) is designed to capture detailed information on the
most severe safety and security incidents occurring in the transit environment.
Detailed data, available from sources such as accident, incident, or police reports,
are used to complete the Reportable Incident Reporting form (S&S-40). One
form is completed for each major incident that occurs at an agency. The information
required on the form is intended to be of a level that can be collected at or
near the time of the incident occurrence. This form is required for all
transit agencies except those filing a Nine or Fewer Vehicles Waiver. |
| The
Summary Security Reporting form (S&S-50) is designed to collect information
on less severe safety and security-related incidents than are gathered on the
Reportable Incident Reporting form (S&S-40). One form is completed per reporting
period that is, per month or quarter, depending on agency fleet
size (discussed in future sections). The Summary Security Reporting form (S&S-50)
summarizes the number of safety incidents that have occurred (such as collisions,
fires, and derailments) and the number of security incidents that have occurred
in a fixed number of categories. Safety elements: This form is required
for all transit agencies except commuter rail (CR) and those agencies filing a
Nine or Fewer Vehicles Waiver. Security
elements: This form is required for all transit agencies except those filing a
Nine or Fewer Vehicles Waiver. |

| The
State Agency Identification form (RU-10) collects contact information for the
state agency reporting. The form includes agency identification information, transit
unit director information, and NTD contact person information. Typically,
the state agency administering the Formula Program for Non-Urbanized Areas (Section
5311) will be responsible for the data collection and compilation from each rural
provider in the state serving the general public. |
| The
Rural General Public Transit Service form (RU-20) collects key financial and non-financial
operating information for each rural general public transit provider. A separate
form is completed for each rural provider of general public transit service within
the state. Typically,
the state agency administering the Formula Program for Non-Urbanized Areas (Section
5311) will be responsible for the data collection and compilation from each rural
provider in the state serving the general public. |
| The
Statewide Summary form (RU-30) summarizes the key financial and non-financial
operating information for rural general public transit services reported on the
individual Rural General Public Transit Service forms (RU-20). Additionally,
this form provides for the reporting of two statewide data items:
- The number of counties
within the state
- The
number of counties served by rural general public transit.
Typically,
the state agency administering the Formula Program for Non-Urbanized Areas (Section
5311) will be responsible for the data collection and compilation from each rural
provider in the state serving the general public. |
